Consent Form
I request to be treated as an Elective Professional Client by FS Investors Ltd (“FS Investors”) and its Principal, Brooklands Fund Management Limited (“Brooklands”). I confirm that I have the necessary expertise, experience and knowledge in light of the nature of the services envisaged, that enable me to make my own investment decisions and understand the risks involved. If I am receiving MiFID II business, I also confirm that I meet at least two of the criteria below:
- I have carried out transactions, in significant size, on the relevant market at an average frequency of 10 per quarter over the previous four quarters;
- the size of my financial instrument portfolio, defined as including cash deposits and financial instruments, exceeds EUR 500,000; or
- I work or have worked in the financial sector for at least one year in a professional position, which requires knowledge of the transactions or services envisaged.
I also confirm that I have read and understood the consequences of losing the regulatory protections afforded to Retail Clients which are set out in
Schedule 2. I understand that, if my request is accepted, it will be my responsibility to inform FS Investors or Brooklands of any changes to my circumstances that could affect my categorisation or if I wish to be recategorised as a Retail Client for any reason.
